The Sub Acute Care program is designed to meet the care requirements of patients with medically complex conditions who are ready to leave an Acute Care Hospital but are not yet ready to go home. Specialized rehabilitation services are provided to help restore patients to their highest possible level of strength and independence. The Sub Acute Care Unit consists of 7 beds. Typically the average length of stay in the Sub Acute Unit is between 15 to 90 days. Coordinated by a Case Manager, a multidisciplinary team involving Physicians, Nurses, Occupational / Physical / Recreational Therapists, Dietician, Pharmacist, Social Worker and Pastoral Care Coordinator work with each patient and his/her family to develop a personal assessment and rehabilitation plan. Prospective individuals for the Sub Acute Unit are assessed by the Alberta Health Services Authority – Central Assessment and Placement Services (CAPS).
