St. Michael's Community Outreach Program
Mission Statement
Dedicated to supporting seniors by educating and connecting them to resources in their community which will meet their various needs, allowing them to live with dignity and independence.
Outreach workers provide information and assistance to seniors in our facilities with regard to:
-
Substitute Decision Making
We provide information, resources and referrals for documents that are important for future planning with regard to personal health decisions, financial decisions and wills.
-
Financial Assistance
We help seniors to access information regarding income support programs through the Federal and Provincial government
-
Elder Abuse
Through supportive listening and information sharing, we assist seniors to identify all forms of abuse including physical, mental, emotional, and financial.
-
Partnering Seniors in the Community
Experiencing loss of home, health, independence and possibly a partner may cause loneliness for many seniors. We provide companionship service that puts lonely seniors in touch with others who may be experiencing similar life transitions
-
Housing
We provide information for seniors who have questions about being ready to move out of their home and how they can get to where they want or need to go.
